The discharge of all financial obligations, completed health records and subsequent health requirements, and health insurance are prerequisite to class and clinical entry.

Current CPR certification at the Provider C level is prerequisite to clinical entry.

Official transcripts for prerequisite and corequisite courses are required for class and clinical entry.

Procedure

  1. Bills are to be paid or a payment plan contract signed to be cleared for class/clinical. The Bursar will notify the Program Director of students not financially cleared.  Students not cleared must receive a class entry slip from the Bursar to enter class after the first day of each semester.
  2. Health records are to be completed before entrance.  The Student and Alumni Affairs Coordinator will submit immunization lists to the Program Director.  Students whose immunizations are not current will need a class entry slip from the Student and Alumni Affairs Coordinator to enter class after the first day of each semester.
  3. Health Insurance Information forms are to be completed before each semester and are filed in the Student and Alumni Affairs Coordinator’s office.
  4. Evidence of current CPR certification at the Provider C level is necessary in order to participate in clinical experiences.  Verification of certification will be done by the Program Director at the beginning of each Medical Radiography semester.
  5. The Registrar will monitor receipt of official transcripts for course exemptions and will notify student and Program Director if official transcripts are not received.
  6. Students will not be allowed into class/clinical after the first day of each semester without the appropriate documentation.
